The Employee Incident or Accident and Safe Schools Incident Reports (SSIR) are found online via the Core through the Report a Hazard/Incident icon and then “submit a new form”. From there, just follow the steps. Advise your Principal of the incidents ASAP and before the end of the day.

Filled out a Safe Schools Incident Report?  You are not allowed to make a copy of this form, however it is recommended that you keep detailed notes for your personal records regarding the incident.
